Document Entra app role values

Summary

• Document the Entra app-role setup required before SCIM role mapping.
• Use the role URL in Organization Settings as the role UUID source, consistent with the Okta instructions.
• Clarify role behavior when supplied values do not match and no default role is configured.

Why

Entra app-role values must match Datadog role UUIDs. Without this setup, Datadog cannot apply the intended role assignments.

• Organization Settings and Microsoft app-role links return HTTP 200
