Fixes #116.

A session's step-up window lets it take MFAStepUp actions (integrations, roles, API tokens and the rest of the
step-up permissions) without a factor for the next 10 minutes. These paths opened one without verifying a factor:

  • a web login that skipped the MFA prompt with a trusted-device cookie;
  • a login whose user needs no MFA challenge, on the web or the CLI;
  • trusting a device, which restarted the window, so a session could keep extending it;
  • enrolling another MFA method, which needed no MFA: a session that never verified a factor could add its own and
    step up with it.

Changes

  • Only a verified factor opens a step-up window (db.UpdateSessionMFAVerified). A login that needs no challenge, or
skips it with a trusted device, is marked verified with ExemptSessionFromMFAChallenge, which opens none.
  • A CLI login records whether it was exempt (new column cli_login_states.mfa_exempt), and the CLI session it
creates gets a step-up window only if it wasn't. A backup code leaves mfa_method_id empty, so that column couldn't
    tell the cases apart.
  • Trusting a device attaches it to the session and leaves the window as it is. The response reports the session's
    real step-up expiry.
  • New MFA level MFAStepUpOnceEnrolled for auth:mfa_method:create: free for a user enrolling their first factor,
step-up once they have one. The web prompts for it before the request, like step_up.
  • Docs: a trusted device skips the sign-in prompt, not step-up; adding a method asks for an existing one.

User-visible change

After signing in on a trusted device, the first step-up action asks for a factor. Adding a second MFA method asks
you to verify an existing one first.

Testing

  • New tests: trusted-device and no-challenge logins open no window; trusting a device keeps the window; an exempt
CLI login's session has no window and a verified one does; the new level for first and additional factors.
  • task go:test:unit (1320 tests), task go:lint, task web:lint:fix and typecheck pass.
  • Web E2E: account-security, mfa-backup-codes, mfa-enrollment-required, login and cli-login-webui (17 tests) pass.

Summary by CodeRabbit

  • New Features
    • Adding an MFA method now requires step-up verification if another method is already set up; setting up the first method does not.
    • Trusted devices skip the MFA prompt at sign-in until their trust period expires.
  • Bug Fixes
    • Signing in without an MFA challenge, or using a trusted device, no longer starts or extends the step-up window. Sensitive actions still require verification when needed.
    • Documentation clarifies when verification is required to add a WebAuthn method and how step-up windows work.

A session's step-up window lets it take MFAStepUp actions (integrations, roles, API tokens and the rest) without
a factor for the next 10 minutes. Several paths opened one without verifying a factor:

- A web login that skipped the MFA prompt with a trusted-device cookie.
- A login whose user needs no MFA challenge, on the web or the CLI.
- Trusting a device, which restarted the window, so a session could keep extending it.
- Enrolling another MFA method, which needed no MFA: a session that never verified a factor could add its own and
  step up with it.

Now:

- Only a verified factor opens a step-up window (db.UpdateSessionMFAVerified). A login that needs no challenge, or
skips it with a trusted device, is marked verified with ExemptSessionFromMFAChallenge, which opens none.
- A CLI login records whether it was exempt (cli_login_states.mfa_exempt), and the CLI session it creates gets a
step-up window only if it wasn't.
- Trusting a device attaches it to the session and leaves the window as it is.
- Adding an MFA method has a new MFA level, MFAStepUpOnceEnrolled: free for a user enrolling their first factor,
step-up once they have one. The web prompts for it before the request, like step_up.

Walkthrough

The changes distinguish MFA factor verification from challenge exemptions during sign-in, add enrollment-aware step-up requirements for creating MFA methods, and prevent trusting a device from changing an existing step-up window.

Changes

MFA step-up behavior

Layer / File(s) Summary
Enrollment-aware MFA method creation
internal/gateway/rbac/mfa_requirements.go, internal/gateway/middleware/mfa.go, internal/gateway/middleware/mfa_stepup_once_enrolled_test.go, internal/gateway/rbac/mfa_requirements_test.go, internal/tools/exportmfa/main.go, web/src/contexts/step-up-context.tsx, docs/src/content/docs/user-guide/mfa/webauthn.mdx
Creating an MFA method now uses MFAStepUpOnceEnrolled. Users without an enrolled factor can add their first method without step-up; enrolled users need step-up to add another. Gateway middleware and the web client handle the new level.
Factor verification and login exemptions
internal/gateway/db/migrations/20261015000000_cli_login_mfa_exempt.sql, internal/gateway/db/cli_login.go, internal/gateway/db/sessions_mutations.go, internal/gateway/auth/session_manager.go, internal/gateway/handlers/auth_helpers.go, internal/gateway/handlers/auth_cli.go, internal/gateway/handlers/auth_cli_complete.go, internal/gateway/handlers/auth_step_up_window_test.go, internal/gateway/CLAUDE.md
Session and CLI login records now distinguish factor verification from challenge exemption. Exempt login paths do not set RecentStepUpAt; CLI completion sets it only for a verified factor. Tests cover the resulting session and login states.
Trusted-device and step-up window handling
internal/gateway/handlers/auth_mfa_management.go, internal/gateway/handlers/auth_step_up_window_test.go, docs/src/content/docs/user-guide/mfa/index.mdx, docs/src/content/docs/user-guide/mfa/trusted-devices.mdx
Trusting the current device requires an existing MFA verification and step-up expiry. It does not change the session’s verification or step-up timestamps. Documentation clarifies that trusted devices skip the sign-in prompt, not later step-up verification.

If attaching a trusted device to the session fails, the user sees an error but the browser may still hold a trusted-device cookie. This is a narrow failure case that can be fixed with a small change. The core step-up window changes look sound.

🔒 Security & Privacy | 🟡 Minor | ⚡ Quick win

Do not return failure after issuing a usable trusted-device cookie.

If session attachment fails, this branch returns HTTP 500 after MintTrustedDevice created the device and setTrustedDeviceCookie sent its token. The browser can then have a trusted device despite the failed response. Set the cookie only after attachment succeeds. Revoke the minted device if attachment fails.
