Scroll-wheel zoom with a modifier (e.g. Ctrl+Alt+scroll)

Zoom in and out with a modifier plus the mouse wheel (e.g. Ctrl+Alt+scroll), the most common zoom gesture in other magnifiers (Windows Magnifier, macOS Zoom, ZoomText). Complements the side-button and keyboard binds.

• Configurable modifier combo in Settings (keybind capture), off or on by default to be decided
• Step per notch configurable; continuous feel (ease between notches) like the existing ramps
• The wheel event must be swallowed while the modifier is held so the app underneath does not scroll (WH_MOUSE_LL, same balanced-swallow rules as the side buttons)
• Works in both engines; respects maxLevel and the MPO walls
