feat(api): add CSRF protection to authenticated mutations — fixes #126

Why

requireAuth (apps/api/src/middleware/require-auth.ts:6) only checks session. Cookie-authenticated POST /forms/:id/publish, POST /templates/:id/purchase etc. accepted cross-origin <form> POSTs. cors({origin: FRONTEND_URL}) doesn't block form submissions.

What

Origin/Referer validation (no token). Closes CodeRabbit comment on apps/web/lib/api-client.ts:16.

Changes

  • apps/api/src/middleware/csrf.ts — new csrfGuard: skip GET/HEAD/OPTIONS + no Cookie, else require Origin/Referer origin ∈ TRUSTED_ORIGINS ?? FRONTEND_URL → 403 otherwise
  • apps/api/src/app.ts:13 — app.use(csrfGuard) globally (after cors/json)
  • apps/api/src/lib/env.ts:28 + apps/api/src/lib/auth.ts:9 — trustedOrigins for Better Auth (/api/auth/* via toNodeHandler)
  • apps/web/lib/api-client.ts:19 — X-Requested-With: XMLHttpRequest hardening

How to test

# should 403 (cross-site form)
curl -X POST http://localhost:3000/api/v1/forms/<id>/publish -H "Cookie: better-auth.session_token=..." -H "Origin: https://evil.com" -v

# should pass (same-origin)
curl -X POST http://localhost:3000/api/v1/forms/<id>/publish -H "Cookie: ..." -H "Origin: http://localhost:3001" -v

# should pass (no cookie — public)
curl -X POST http://localhost:3000/api/v1/forms/<id>/responses -d '{}' -H "Content-Type: application/json" -v

Env
FRONTEND_URL required, optional TRUSTED_ORIGINS=http://localhost:3001,https://prod.com (comma-separated).

  • Prevents cookie-authenticated cross-origin mutation requests.
  • Validates trusted Origin or Referer values.
  • Rejects invalid requests with 403.
  • Adds trusted origin configuration for Better Auth.
  • Adds the X-Requested-With header to web API requests.

📝 Walkthrough

Walkthrough

The API now derives trusted origins from environment configuration, passes them to betterAuth, and applies CSRF validation before routing. The web API client sends X-Requested-With on all requests.

Changes

CSRF protection

Layer / File(s) Summary
Trusted origin configuration
apps/api/src/lib/env.ts, apps/api/src/lib/auth.ts
config.trustedOrigins resolves comma-separated origins with fallbacks. betterAuth receives the configured origins.
Origin validation middleware
apps/api/src/middleware/csrf.ts
csrfGuard skips safe methods and cookie-less requests. It validates Origin or Referer against normalized trusted origins and returns 403 responses for invalid requests.
Application and client wiring
apps/api/src/app.ts, apps/web/lib/api-client.ts
The Express app registers csrfGuard before routing. apiClient sends X-Requested-With: XMLHttpRequest on every request.

Merge Risk: 🟡 Moderate · up to 7edb9

The PR adds global origin checks for cookie-authenticated mutations, but an empty TRUSTED_ORIGINS setting can cause every such request to return 403, while differing CORS and trusted-origin settings can block approved browser clients. Merge should wait for configuration fallback/validation or explicit owner acceptance.

Explanation

The PR satisfies issue #126 by adding global CSRF middleware for custom API routes, validating Origin or Referer on cookie-authenticated mutation requests, and returning 403 for invalid origins. Safe methods and requests without cookies are skipped.
