Fix Codex Locality MCP startup authentication

Summary

• configure Codex to launch the Locality MCP bridge over stdio instead of duplicating the daemon HTTP bearer token
• separate HTTP and stdio MCP configuration discovery so loc mcp does not create or require an HTTP token
• update MCP guidance and add regression coverage for stale authenticated HTTP config migration

Verification

• cargo test -p locality-desktop agent_guidance::tests -- --nocapture
• cargo test -p loc-cli --test mcp -- --nocapture
• cargo test -p localityd mcp::tests -- --nocapture
• cargo fmt --all -- --check
• git diff --check
• verified the installed loc mcp initialize handshake and Codex stdio configuration locally
