fix(create_assessment): clarify that name or assetId is required alongside template

🎯 What does this PR do?

Updates the create_assessment tool descriptions to reflect that the API requires at least one of name or assetId in addition to template:

• Package comment: clarified that both template and at least one of name or assetId are required
• name field: updated from "Optional" to "Required when assetId is not provided"
• assetId field: updated from "Optional" to "Required when name is not provided"
• Tool Description: updated to state that the API rejects requests that supply neither name nor assetId
